Disqus, my super-simple, direct advice to you: Fix real-time updating so that it works 97% of the time. If you cannot do that, stop trying, AND ditch nested replies.
Hundreds of commenting systems have worked well without either. None have worked with only nested replies.

Normally, I can adapt to comment systems, but I'm really frustrated with Disqus in its latest incarnation. I've been unable to use it nearly all week.

Well, really good comments can make up for a lot of deficiencies in the commenting system. There's no denying that.
